Application Form to the Data Controller

APPLICATION FORM TO THE DATA CONTROLLER

(Using this form is not mandatory. The form is provided as a supporting document for data subjects in the application process; every application made in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller will be assessed.)

General Notes

Under Article 11 of Law No. 6698 on the Protection of Personal Data ("PDPL"), every person has the right to apply to Sesli Tekstil Sanayi ve Ticaret AŞ (the "Company") as the data controller on the following matters:

• To learn whether personal data is being processed,
• To request information about processing if data has been processed,
• To learn the purpose of processing and whether the data is used in accordance with that purpose,
• To know the third parties, domestic or abroad, to whom personal data has been transferred,
• To request correction of personal data that has been processed incompletely or incorrectly,
• To request the deletion or destruction of personal data — ex officio or upon request — where the reasons for processing have ceased, even if processed in line with the PDPL and other applicable laws, under Article 7 of the PDPL,
• To request that the actions taken under (e) and (f) be communicated to third parties to whom the personal data has been transferred,
• To object to outcomes generated against the data subject through analysis carried out exclusively by automated systems,
• To claim compensation for damages arising from the unlawful processing of personal data.

Processing of Personal Data Related to the Application

Through the Application Form to the Data Controller, personal data (full name, Turkish ID number, passport number, phone number, e-mail, address, signature) is processed in a limited and related manner for the purpose of identifying the applicant and managing the request. Where necessary and depending on the nature of the request, this data may be shared only with the relevant internal units/persons and legally authorised bodies/institutions.
